- Learner's birth certificate
- A recent photo of the learner
- Certified ID copy of the parent or guardian
- Proof of address (utility bill, lease, or bank statement showing address)
Depending on your situation
- Immunisation card and most recent school report, if applicable
- Medical aid card, if the learner is on one

Income documents for whoever pays the fees — the exact list depends on employment status,
for example payslips and bank statements if employed, or business records if self-employed.
The form will ask for the right ones once you get there.
Files must be PDF, JPG or PNG, and under 5MB each. If a document is on paper, a clear photo works fine.
